Most people never read their own records, and those who do have few options when something is wrong. Correction processes are slow where they exist at all, and the patient's own account of a visit usually has no place in the record. Meanwhile, what one doctor writes becomes the starting truth for the next.

You can actually see your record, all of it, and read it in plain language. When you find something wrong, you attach a correction as a clearly labeled note of your own. The original stays exactly as written, because a record that can be silently edited is a record nobody can trust. What changes is that your account now exists alongside it, visibly yours, with its own date.
Every fact in the record carries a verification tier: fetched from a provider system, extracted by AI from a document with the original attached, or noted by you. A doctor reading your record can always tell which is which, and that distinction is exactly what makes the record safe to rely on.
On your terms
Your notes are part of your record, and you decide whether a given share includes them. Because provenance is preserved, including them never muddies the water: a reader always sees what the source said and what you added.
We are also designing for a future step where a clinician can review and confirm a patient note, so corrections can graduate into the trusted record. That step is direction, and it is honest to say it is not built yet.
